Tesamorelin

▸ Overview

Tesamorelin is a stabilized GHRH analog approved by the FDA to reduce excess visceral abdominal fat in patients with HIV-associated lipodystrophy. In research it is also studied for its impact on body composition and metabolism.

▸ Mechanism of action

• A stabilized GHRH analog (with trans-3-hexenoic acid at the N-terminus) that increases its half-life and stability.
• Acts on pituitary GHRH-R receptors, stimulating the pulsatile release of GH.
• Induces a significant reduction in visceral abdominal fat through the GH→IGF-1→lipolysis axis.
• Lowers plasma triglycerides and improves the lipid profile in clinical studies.
• Unlike exogenous GH, it preserves feedback regulation, reducing metabolic risks.
• It is the only FDA-approved GHRH analog (Egrifta) for lipodystrophy in HIV patients.

▸ Administration

• Subcutaneous route: injection in the abdomen, rotating the site.
• Reconstitution: lyophilized powder with the supplied diluent or bacteriostatic water.
• Storage: strict refrigeration before and after reconstitution.

▸ Timing

• Usually administered at night, before sleep.
• At least 2 hours of fasting recommended to optimize the GH pulse.
